BAILEY, GENERAL MANAGER SUBJECT: RECEIVE UPDATE ON COMPLETED CHANGE ORDER FOR EMERGENCY WORK AT THE TREATMENT PLANT (PRIMARY SEDIMENTATION TANK REPAIRS) On August 3, 2017, the Board of Directors declared an emergency and allowed the General Manager to execute a change order with W.M. Lyles Co. based on the time necessary to complete the repairs on the Primary Sedimentation Tanks (PST) prior to the wet weather season. The following repairs were completed: • Replaced floor rails and flights in PST No. 1; • Replaced floor rails, chains, and flights in PST No. 2; and • Replaced chains and flights in PST No. 3. All work was completed by October 19, 2017. The estimated budget for the work was $800,000, which included contractor costs, testing, staff time, and contingency. Attached is a presentation of the critical work performed and the final change order and budget to date. ATTACHMENTS: 1. Lopez, PE ,,,:_ — Capital Projects Division Manager PRIMARY SEDIMENTATION TANK (PST) REPAIRS Background: • Three major rail failures have occurred in the past year: • PST No. 4 failed August 2016 • PST No. 1 failed January 2017 • PST No. 1 failed May 2017 • All four PST's were inspected and staff concluded that major repairs were needed • Request for Proposals were sent to three Contractors that were currently working on Central San Projects. Two Proposals were received; W.M.
