At the April 25, 2017 Finance Committee meeting, the committee requested additional
information on the following check expenditures. Below is staff's response:

#214437 $37672.43 Hertz Chair Nejedly asked if the generator that
4/13/17 Equipment was rented was used; the invoice showed
Rental zero hours of usage.
Check #103362 was issued to Restoration Management for an overflow on El Camp anera
in Orinda that took place on March 14, 2017.
Check #103363 was issued to Servicesmaster Management Co. for an over-
pressurized line on February 3, 2017 and again on February 7, 2017 for an overflow on
Rahada Place in Lafayette. These two charges are also included in the Loss Control
Report presented to the Administration Committee.
Check #214042 was issued to U.S. Health Works Medical for the annual medical
surveillance required to be provided for employees in the respiratory protection
program. There are two line items that were paid with this check. The first one is the
cost to review respiratory medical questionnaires, and the second is for follow-up
appointments required based on questionnaire responses. This

is part of the medical surveillance requirements from CalOSHA, and the cost is included
in the Risk Management Budget.
Check #214437 was issued to Hertz Equipment Rental for the rental of an emergency
portable generator to replace an existing generator which had to be taken out of service
for regulatory reasons. The California Air Resources Board requires all engines rated at
50 horsepower or greater to meet Tier 4 emission limits as of January 1, 2017. This is a
temporary rental until the purchase of a new one. A new generator is in the process of
being purchased, but must be sized correctly for its intended use at multiple pumping
stations. Generators over 50-hp must meet Tier-4 final level, which is not readily
available for all size categories and voltages. The new generator must also balance
portability and performance. Because of these constraints, staff has been testing
different generators, and are getting quotes. Unless there is an emergency, this
generator will not have run time on it. It has been tested at the sites where it will be
needed.
